Notes & limits

• Covers up to five standard-complexity forms; oversized forms, multi-language, or custom API/deep integrations can be added as line-items.
• HIPAA alignment depends on your hosting & plugin configuration; this service does not provide legal compliance certification.
• Does not include content writing from scratch beyond field labels and helper text (we can polish your provided copy).
